Final Forms are now available to complete for the 2026-27 school year. To get started go to https://columbusgrove-oh.finalforms.com and sign in with your parent account. Click on the blue button at the top of the page:
Select any activities that your student may participate in, update any information as needed, and sign each form. Forms must be completed in the 2026-27 year before a physical form can be printed out. 

Athletic physicals will be given by Grove Chiropractic on Tuesday, May 12 from 3-6pm for athletes in grades 6-8 planning to play sports next year. Physicals for students going into grades 9-11 will be given on Tuesday, May 19 from 3-6 pm. The cost will be $10.
